PORTFOLIO COMMITTEE ON HEALTH AND WELLNESS NOTES HEALTH OMBUD REPORT ON MENTAL HEALTHCARE

The Gauteng Provincial Legislature’s Portfolio Committee on Health and Wellness has noted with serious concern the findings of the Health Ombud’s report into the care and death of Ms Mohlamme at Dr George Mukhari Academic Hospital. The report points to significant systemic failures in mental healthcare delivery, patient safety, and governance. The Committee extends its condolences to the family of Ms Mohlamme.

The Committee notes that the incident occurred in June 2024, a period which prior to the current term of the committee was at a transitional stage following the 2024 general elections.

In line with proper oversight processes, the Committee will first engage the Health Ombud to ensure a clear understanding of the report and its recommendations. The Gauteng Department of Health will also be invited to participate to ensure transparency and alignment on the implementation of remedial actions.

Chairperson of the Portfolio Committee on Health and Wellness, Adv Ezra Letsoalo, said, “We take these findings seriously and will ensure that they are fully interrogated. Our focus is on strengthening accountability and improving the safety and quality of mental healthcare services in Gauteng.”

The Committee acknowledges that the findings of the Health Ombud reinforce and extend its ongoing oversight work, including the focused intervention study on mental health services across Gauteng, which commenced in November 2025. This process included oversight visits to Dr George Mukhari Academic Hospital and several other facilities across the province. The work forms part of the Committee’s broader efforts to address systemic challenges and improve patient outcomes.

The Committee assures the public that it is seized with the matter and remains committed to ensuring that the Health Ombud’s findings are acted upon to strengthen the quality, safety, and governance of mental health services in the province.
